I need help and a part

I just bought a fast hitch assembly but I didn't notice the hand crank depth control where it bolts to the finals has been cut off. Can anyone make me a cardboard template so I can repair

Also I'm missing the trunion for the lift arm. Any help would be greatly appreciated
