Voluntary Self-Identification of Disability

Employer keeps sending me this form to fill out. Form says it is voluntary. Note on envelope says 'must sign'. I have ignored and not filled out 3 previous requests. Anyone here familiar with this form? I realize it's pretty harmless, but I have a problem being told to fill out something that is supposed to be voluntary. I will research when I can

I think it means that if you fill out the form, you must sign the form.

Query the HR department. Also ask what they do with the information. My guess the information is utilized in employer mandatory reportable statistics.
