Here's the problem: viewtopic.php?f=1&t=91806 And here's what I used to get it out, a small pointed chisel/hammer and angled needle nose pliers. Fortunately it was near the top of the hole and I backed it a couple of turns with the chisel and hammer then the pliers could catch it and turn it on out.
