The bushing I used was from a good hardware store, seems to fit perfectly. I have been using it for a few years now and the only 'problem' is it can easily go missing.

Unless you have a need to remove them, leave the blocks in the square tube.

Not sure, but think I remember 1/2 X 2 bar nine inches long. Hole on each end one on center and one between to match torque tube. Should be 5/8ths holes on center at one inch, three inches, 4 inches, and one inch on center. Hope I'm making sense.
